How do RPG developers get the experience/leveling scaling right?

I was recently playing Chrono Trigger, and the pacing felt really good. I never had to grind, and always felt like I was at the appropriate level for whatever I was facing.

How do they pull that off? Do they just tweak it through copious testing? Are there known tables or equations or something it's based on?
