Why is the no universal system for medical information yet?

My pharmacist can look up all the prescriptions I've ever had, but I need to transfer records between doctor's office. It just seems like this should all be connected by now.

Note: I think this is true about prescriptions. I used to get my meds filled at Costco (10 years ago) and my new pharmacy was able to see that. I can't imagine Costco pharmacy is associated with my grocer's pharmacy.
