Is it difficult for construction companies to avoid water lines?

In the last 3 months Iโve lived in this town, twice has the cities main water supply been hit by construction, and today the campus of the school I go to had its water line broke. I know nothing about digging/construction, but Iโm trying to view it from a place of sympathy since itโs quite annoying having to go out and buy bottled water when this happens.
Is it difficult to know the precise location of these pipes? Is it difficult to control the equipment to prevent damage?
