How do you say "no" when someone crosses your boundaries or you don't want to do something that someone asks?

When people say to learn to say no, do they mean you literally say the word "no"...or do you soften the blow a little bit by saying "sorry I can't do that" or "no thank you" or "that is not something I can do right now"?
