Why don’t our presidents have heart attacks?

I’m thinking specifically of Biden and Trump due to their age, but this applies to old leaders all over the world. We see the physical effects off all the stress on these people, but they don’t have heart attacks (publicly) like you might expect from old people under huge stress. Are they getting amazing drug cocktails? Are they beyond different? Is it still statistically unlikely to happen?
