Are waterline breaks mostly the result of careless ineptitude, or is it a legitimately honest mistake?

Our little town of about 3,500 people is under a boil order for the second time in less than a month because a crew was digging and broke a water line.
I can’t remember a time when we’ve had two boilers so close to each other. It made me wonder, is this something that is just a genuine risk that any crew takes when doing maintenance? Or is it something dumb that could have been prevented?
