11 year old can't tell left and right?

My little brother is 11, and for some reason he cannot tell left and right. Like for example we just moved and I was explaining to him where the local 7/11 was, and I said something about going right, he did that thing with his hands where your left hand makes an L shape, then proceeded to STILL point the wrong way. Besides this he's pretty smart, he's better than I am at math (I'm 19,) reads at grade level, etc, but can't remember two directions? I had it down far earlier than he did, to the point where I don't even remember learning it, and the same goes with everyone else I live with.
