if sewage is managed with slopes/gravity, before modern technology, how did nations with flat lands manage their waste without slopes/gravity?

as the title said. Yes, I know a lot of nations had terrible waste management and threw waste into streets/canals/their own waters.

I'm referring to places like steppes. where there wasnt much water/canals, the land was very flat and dry. and im specifically referring to nations that knew the risk of sewage strung about and wanted to minimize harm. what did they do? (Note: im referring to pre-modern technology so no intricate pump systems that wouldnt have existed in the early 1800s)
