How does Medicare or Medicaid coverage varry by state if it is a federal thing?

I know Medicare (typically for the elderly) and Medicaid (typically for the lower class; people with disabilities) are both federal programs. How is it then that someone who I know who moved from Minnesota to Arkansas suddenly gets different coverage for both? If it is a federal program how does it make any difference if you move from state to state? Plus what state would be the best for these programs like Medicaid for the individual?
