In practice, how often do people use the dictionary definitions of words?

In conversations as well as online posts, it is common for people to use words without thinking, and often leads to unintended misunderstandings due to poor choice of words when not explained. In practice, do people ever actually use the structure dictionary definition of words or is it rare to do so? When talking, should it be assumed by default that people don't strictly mean when they use certain words?
