Are First Responders Trained To Respect Or Ignore A Patient Refusing Treatment In An Emergency Scenario (In The US)?

So when you are listening to people recalling near-fatal accidents, attacks, etc you will sometimes hear them say "and I told (the paramedic) 'please let me go! Go help (other victims)! Let me go! No!'... And then I woke up in the hospital..." I understand that you can have a DNR, but if you are conscious enough to refuse treatment do you really have no right to refuse life-saving measures? You could fully understand that your life will be full of pain, medical debt, substance abuse, psychiatric issues, etc and you have no right to refuse to go on?

I can only find things about right to die in the scenario of being in a vegetative state and the family "pulling the plug", but nothing about "this is me explicitly saying I DON'T WANT TO SURVIVE THIS"
