What was a non-smoker's life like in mid 20th century US/Europe?

Films and historic accounts from that period suggest that smoking was absolutely ubiquitous, from diners to airplanes to schools to hospitals.
How did non-smokers survive in such environment? How strong was the pressure to start smoking yourself? What were the ways to protect yourself?
