Been flatlining for years, trying to conquer severe PIED. Any thoughts?

So, let me set the scene. I've been attempting streaks since the summer of '15. Mostly all of my relapses have been MO, not P, especially within the last year. My streaks have extended from about a week or two in the beginning months to about 50-60 day streaks in the past half a year. Been with a SO since the beginning as well, and we recently tried jump starting but to no success, so I'm now on monk mode as of the beginning of this month. Just curious as to anyones thoughts who's been through similar? I've done NoFap once before when I was 18 and conquered the PIED I'd developed, but I was single and busy all the time so sexual imagery/stimulation was pretty much absent. My assumption is that I need to recreate that environment and stick with it in order to recover, but what do you all think? I know the problem isn't organic, tested last year with P, and besides that I'm about as in shape and healthy as a human being can get naturally. I also don't really suffer from any flatline symptoms other than ED. No brain fog, no social anxiety, no depression or anything like that. I feel great - I just can't get a boner :p (weak morning wood as well, when I do have it).
