When you came up with your name did you keep your last name?

I just want to know if people also make their own last names or just keep the one their family gave them?
What about ppl who no longer have contact with their family? Did you still keep your family last name?
I am in a bit of a situation were I believe that my family would disown me if I came out to them. I am still in the closet about everything but i am 21 and will graduate college next year. But I honestly wouldn't mind figuratively running away and starting a new life if my family didn't want anything to do with me. But i don't know what to do about my last name if that happens.
