My drs are not listening.

This is a long one so thank you to anyone that invests their time into listening and making any suggestions.

5 years ago I was 305lbs, for context Iā€™m 6ā€™4 and very broad and well built. Today, I am 555lbs and I am in so much pain I can barely walk. Let me check all the checks for everyone first.

-Sub 2000 calories daily -Work in a physical environment with heat all day welding. -I adhere to strict time restricted feeding 12pm-8pm so I get a 16 hour fast daily -I do not eat carbs or processed sugars I donā€™t drink sugary beverages -I donā€™t drink alcohol or use any other substances. -I get over 7,000 steps a day in on average when Iā€™m able to be fully active. -high fiber high protein diet essentially time restricted keto religiously for over 5 months.

I have had complete panels done when I say complete I mean complete. Liver kidneys heart thyroid ct scans with contrast a1c, testosterone, stool samplesā€¦.

My labs are ā€œwithin rangeā€ my a1c is 4.5 I keep an eating journal for my drs and I showed them the data from my Apple Watch.

Hereā€™s where I present things that donā€™t make sense beyond those things. To be active at 555lbs the amount of calories it takes to move alone is far more than 2k calorie consumption a day. So essentially my car is running on empty but somehow still going and inventing its own fuel. They agree itā€™s severe metabolic syndrome but according to their calculations I would need to store 3500 calories a day in order to gain weight at the pace Iā€™m gaining weight.

My free and total testosterone are low but thatā€™s normal for obese people essentially my testosterone levels are diluted due to mass.

I track blood glucose levels and have used 3 different monitors. My blood glucose at the end of my fast is 140s an hour after I eat it drops into the 90s completely ass backwards.

My endocrinologist put me on ozempic after 3 months of being on ozempic my a1c went to 7.4 mind you my diet hasnā€™t changed. I donā€™t even like sweets Iā€™d take a steak and baked potato over a brownie any day.

People who are educated are going to look at the numbers Ive given and say this isnā€™t possible the math doesnā€™t math. I agree so do my drs. I am waiting to be sent to a specialty hospital for further research on my condition but Iā€™m hopeful someone has experienced something similar and possibly has insight or suggestions.

Please dont hesitate to ask any questions or give any insight or suggestion.

P.S. I do have periods of time that my body hurts so bad I canā€™t physically deal with the pain and it takes 3 days generally to recover. Also when my pcp prescribes me prednisone I feel like a million dollars my symptoms align with lupus or a connective tissue disorder and I believe that I would be able to lose the weight if I was able to be consistently active but when Iā€™m dormant I lower my calorie intake to less than 1k a day.
