I'll go first. I left a loose bolt in the timing belt compartment and it tore the belt up after startup🥲. Engine was fine but I had a heart attack being my first timing belt job. Still have the belt in my workshop hanged up as a decoration.

Another time I didn't know I dropped a bolt in my riding lawnmowers carburator. It got sucked into the engine and completely destroyed the cylinder
