losing my mind trying to create a database. need help

I'm trying to create a database but having difficulty, hence, this post. I want to make a database that, I suppose, acts like an inventory or recipe database, that has a field for a single text based entries, but then also has, I guess, another form/table embedded in the form to add multiple lines of text, unique to that particular record. I have created multiple tables for each form I wish to have data entered in, but my difficulty is getting the multi-lined forms to update (essentially clear) for the next record to be entered. I'm not sure if I'm being very clear in my description. If anyone has any idea what I'm talking about, need a better description, or has any kind of help I'd appreciate it.

*edit: the fields I am dealing with are related to insurance arrests. I have a field for "Venue", "Insurance company" and the person "arrested". the fields with multiple entries deal with the associated medical "clinics" and the "offenses" they were arrested for.

*Edit2: had a brainstorm last night: i was adding additional subforms & having issues. I tried it with datasheets instead and think i am getting closer to what it is i needed. now i'll have to configure the datasheets the way i need & figure out how to set up the relationships so i can run reports (maybe not using multiple tables?
