Why do many Cardiologists take forever to CCT? Is the training pathway really a slog? Is an MD required as a minimum to progress

This may just be my experience but almost all cardiology consultants I've come across have taken well over 10 years to CCT? Most also have MDs minimum and some PHDs which makes me think if this is the reason for delayed CCT?
Is an MD required as a minimum to progress and get a NTN?
Cardiology seems a great career but also an absolute slog to get to CCT.
I know many specialties are like this but they don't seem to have consultants with MDs and PHDs as readily as cardiology does
