How do jazz musicians play tunes?

By this I mean Right before a trio plays do they whisper like "II-V-I 120bpm" and they just take off from their or is it more like learning sheet music music you play a I-IV-VI-II for 5 measures and you have to memorize the chord/key change and the only improve going on is with the notes. I know it seems a little strange to ask but I'm a self teaching musician and ever since I started always thought jazz was just pure improve to a set progression for the whole song and trios just could feel when to turn the song around or am I just thinking of free jazz?
