How do I send a text based self-evaluation over email?

So I applied for a job and in the email I was told to write a self evaluation based on a list of technologies. I've written it I guess you could say I'm not sure how to actually execute it. I also need to attach my resume because they wanted to see a Microsoft Certification Transcript first.

Should I start out the email saying hello, then tell the person I've attached my resume and the evaluation is below, say hope to hear from you soon and sign my name, then a few lines down have the self assessment?

This is my first time doing an assessment like this and not sure how to execute it. I feel like just plopping the assessment right before saying "Hope to hear from you soon" wouldn't work well from the flow and maybe how I structured it.
