How do you help my dad apply to be a recipient?

he’s over 65 so wanted to help him apply to be a recipient so someone or me can take care of him? What’s the process? I tried calling the generic number and no one picks up and make you wait in the queue.
Is there ways to fill out the form online and if I go to the office do I need to take my dad with me?
