Hypo/hyper-like symptoms, TSH normal but symptoms still severe

Post Body

Hi everyone. I'm a 26M, currently on a year abroad in Japan for another few months. I've struggled with hyper/hypo-thyroid-like symptoms for 7 years or so now. This is everything I can think of that may be connected:

- Throbbing headaches and thyroid pain

- Constant fatigue

- Heaviness behind eyes

- Brain fog/memory issues

- Insomnia - either not being able to fall asleep (currently taking zopiclone for this and it usually helps), or I wake up in the middle of the night constantly and sometimes can't fall asleep again. This still happens frequently

- Heart palpitations, never been deathly alarming but these days it often feels like it's beating noticeably harder than it should

- Excessive sweating, especially from armpits

- Overly frequent need to urinate

- Constipation

- Slightly high blood pressure for someone my age according to GP tests over the last 1.5 years

- Potential body temperature issues, I'm often saying I'm too hot or too cold when my girlfriend feels the opposite. Being cold in the middle of the night is particularly common

When these symptoms first started, it was mostly the headaches and fatigue and I assumed they were due to a lack of sleep/general tiredness, so I would go to bed early and they'd pretty much go away. These days, they're always there to some degree and all I can do is try to minimise them. The only thing I've found that has any positive effect on my symptoms is, seemingly, good quality sleep. Which is very difficult for me to get consistently due to my aforementioned insomnia. Emphasis on good quality sleep here, as very occasionally I can wake up after 6-7 hours of sleep and miraculously feel OK, but on average I sleep for 8-9 hours and still wake up feeling like crap. On some days, the pain in my head and neck is so severe that I try to nap during the day for an hour which often improves it slightly. I also have ADHD (recently started medication, not helping much) and anxiety so they may definitely contribute/be the cause for a few symptoms, but overall it is obvious there's something else wrong.

Recently, I had a blood test to check for Hashimoto's disease. You can view the results (TSH FT4 something I can't translate thyroglobulin) here, which according to my GP are all normal. Was just wondering if anyone had some advice on where to go from here, or any potential further tests. I'm now basically fatigued and in pain on a daily basis and it's ruining my life so I'm getting desperate. Some days I can barely even get out of bed at all.

I have to come up with the next step myself as my GP frankly does not inspire confidence, and doesn't seem to have any ideas whatsoever unless I suggest them. His hypothesis is that this all just a result of stress related to my ADHD, which is fucking baffling as I never told him I was stressed in the first place, plus back when these symptoms started I was simply sitting around at home doing nothing with my life.

In August I'll return to the UK where it'll be much easier to have access to decent English-speaking doctors, but if there's anything I can do now I really think it should be done. I can also get blood tests done on the spot here whereas at home I usually have to book and then wait a week or two.

Thanks in advance.
