The premise of this one is to have a character using the literal circles of hell to attack with. As Azazel, start with Blue Cap, Dead Onion, The Ludovico Technique, Athame, Circle of Protection, Maw of the Void, Black Powder, and Soy Milk. Your goal is to transform into a "Leviathan" and overthrow "The Lamb", becoming the new king of hell. Rules are that you cannot pick up soul hearts or eternal hearts, only red and black hearts. You can only take devil deal items and health upgrades, as well as anything that gives black hearts upon picking it up. And you cannot buy anything from shops or give to any beggars (that aren't demon beggars) GL HF. Love your stuff.
