Pellet Hops - Best Practices

So far I've just been adding pellets right to the boil and right into the fermenter if I'm doing a dry hop. By god they really add up if doing a hoppy style. So much sludge ends up in the fermenter I get stuck transfers and it's just generally a pain.

What are your tips? I have some muslin bags but I'm not sure what the best way to sanitize/use them is. Do you get the same hop utilization with a bag that you would if loose? Will I have to compensate with my recipe?

Edit: I also bought a hop spider but it got clogged so quickly it didn't look like the hop water was mixing with the rest of the wort. And it's a pain in the ass to clean.
