Big driveway project (regrading, paving, utilities run to detached garage underneath) - Need advice!

- Located in Pittsburgh, PA

Current situation
- My driveway is currently crushed gravel
- Existing driveway grading is less than ideal
- The detached garage currently has 40A service fed via a direct burial line from the house
- No other utilities exist between the house and the garage (other than copper phone lines)
What I want to have done
- Regrade the driveway to make it a bit easier to enter/exit with low vehicles, and easier to use when it snows
- I want to have the driveway paved with asphalt
- I'd like a natural gas line, water line, upgraded electrical service line (80A?), and conduit for low voltage network cabling installed under the driveway to feed the garage
Questions
- Who should I have bid this? An asphalt paving company? A general contractor? Act as a general myself and have different companies bid each piece?
- How do I effectively determine/communicate the driveway grading I'm after? I really just want it to be level at the top, gradual enough that I won't scrape in a low coupe, and have less trouble getting out of the driveway when it snows.
- Any advice/problems you see?
- Should I type up some kind of document so everyone is on the same page as far as expectations?
- What should I be expecting, cost wise?
