Kingdom's Edge battle, BTW. One where I might have had more trouble that it was worth due to my reliance on nail arts (mostly Great Slash, only sometimes Cyclone Slash) and Shade Soul (and refusal to use Weaversong, as that was a bit too powerful).
She disappeared and I thought that meant she simply stopped rendering and therefore I'd have a fun little death involving getting killed by an invisible Hornet (or an easily solvable softlock). Instead the achievement just popped up and she was there.
I think it might have happened while she was next to a gate, but I'd like to know if someone has this better documented.
