95% of pain in butt/Piriformis area?

Diagnosed with an anterior labral tear CAM impingement. Pain for about 2 years up until this. High level combat sport fighter turned gym owner.

Only thing is 95% of my pain is in my butt area / piriformis. Orthopedic surgeon strongly recommended immediately surgery, but of course they have an incentive to cut. PT thinks it might be old nagging injury from a hamstring/glute tear.

Been going on 3 months of very intensive PT with pain immediately coming back when I try any return to sport. Basically dropped out all athletic stuff from running to training to biking etc.

Anyone have similar pain experiences? I don't want to go under the knife and go through this long recovery if the pain isn't even coming from the tear and impingement...
