First dermatologist appointment!

Hi everyone! I’ve got my first dermatologist appointment tomorrow morning for a cyst/abscess on my panty line in the groin that I’ve had for around 6-7 years. I’ve got my period and it’s also really inflamed and kinda grown. I think it had tunnelled as another follicle nearby was leaking some fluid and then it feels like they’ve fused together. I’ve got a list of all my previous medications that GPs had provided mainly antibiotics that did not work. I’ve also got photos over the years. And also have a list of all the products I’ve tried and routinely use like glycolic acid, Vicks etc.

My concerns are: - I really don’t want to leave the appointment being prescribed antibiotics either oral or cream because both have not worked with different antibiotics and given me thrush every time. - I’ve had this thing on me for so long although haven’t been diagnosed yet maybe tomorrow, I want it taken off my body. Is deroofing something you need to qualify for or if I ask I could have it done? - I really just want to make the most of this 20-25 min appointment so please advise me on what to say what to ask for anything I should be aware of. Please any advice will be highly appreciated!

for your sake, I really hope it isn't HS.

Hormones effect the HS flares... Inflammation in your body in general causes acne/irritates current acne..

I noticed that my HS and my cystic facial acne was definitely worse around my period. This is where the BC and Spiro come in.. They work ridiculously well in tandem to help me.

Doesn't have to do with the heat with the underwear, no I wouldn't say that...

I've found that as a woman, I have to be Pushy to get what I want in a doctors office.. I have to insist on treatments. You can't have meek answers/statements either... No "well kind of". It's fucking annoying. But damn if it isn't effective.

I would also make sure you have a follow up appointment (at max 6-8 weeks if it's HS)..

In my case the shots of cortisone actually did drastically calm them down (as well as some topicals) and I'm not actively working towards surgical removal anymore, so somethingsomething doctor was right and all that.

Yeah, I second this OP. I know de-roofing sounds like the best option initially but corticosteroid shots are very effective.

Even if you remove the spots where you flare, you have to take a chunk of flesh out... It's not even a "last resort" method imo because the cons outweigh the pros imo... There are posts and photos of people removing the cysts entirely on here..

so you've not yet been diagnosed with HS?... you're only describing one particular abscess.... it may just be a case of acne..? idk assuming it IS HS:

antibiotics can only do so much... I personally have loved taking spironolactone and a birth control (rated to treat acne...) both prescribed by my Derm. there are only a handful of BC that are rated to treat acne (mine is Ortho Tri-Cyclen)...

also --- whenever you get prescribed antibiotics the doctor can give you a pill for the thrush/yeast infection.... I do that every time I get sick and see my GP/PCP.. Push for the things you need, seriously.

Can you get it de-roofed? mm... maybe?

It would make more sense to inject it with a corticorsteriod (spelling?) so it deflates... I will go and get my abscesses injected with steroids...

Why don't they deroof / want to cut them out? It's kind of an extreme option... and it doesn't mean they won't come back. HS tunnels under the skin via the sinus tracts... so if you want the Nuclear option you'd have to have an appt just to cut out all the tracts. But again... if you have HS all it takes is a hair follicle. we're covered in hair. so... yeah.

Not all Derms are the same. Plenty of them are not familiar with/do not treat HS.

I'm going to toss in that pretty much everything I've said you can find within this subreddit and/or google for confirmation...

You really want to leave the appointment with prescriptions imo... and a follow-up appointment. I saw my derm once every 5 weeks until we got the HS under control/kicked into remission.
