Game audio cuts out when plugging headphones in

I’m curious if this is a me issue or if anyone else has a problem with this. I’m on Xbox and if I start the game up with my headphones plugged in, things work fine. If I plug them in after I start the game up, I can hear voice chat but nothing else. If my controller disconnects while playing, the same thing happens…no game audio but voice works. Strange.
