Work/an alarm makes sleeping impossible and I don't know what will help

I just got done with around having a 10 or so day total vacation for the holiday. I stayed up late, maybe to midnight or 2 am, and slept in usually until 8-10 am. I'd naturally wake up and not wake up in the middle of the night. I didn't have the best sleep, but it was pretty good and it was very consistent.

That changed, predictably, when I started working. I have an alarm set to 6 am. I fall asleep around 10-12 or so, and find myself waking up 2-4 times throughout the night. It's currently 4:30 am, I woke up around 4.

I fall asleep while driving and while at work. Even on days when I get longer hours of sleep, this is the case. It feels like almost no matter what I do, if the next day means there is work, I cannot sleep.

This was the case when I worked overnight, from like... 8 pm to 6 am. I thought the night schedule would help, because I liked being up at night, but it didn't. Same problem, I would be tossing and turning (I had blackout curtaind and everything) and would fall asleep on the road and at work.

I have an appointment with a doctor for this, but because the doctor can only see me during the same hours I work mostly, I had to schedule over a month away, rather than a few days.

I just wish I could sleep. I hate that my sleep gets disturbed while sleeping, so I can't do anything about it, because I'm unconscious. I just have to hope and pray that I get good sleep on a day, and don't crash driving to work.

I don't know what to do.
