What would be a balanced downside to a choose all of the above ability?

I've been theory crafting a Hades Keepsake and one idea of mine was that 1/2/3 times a run you could choose all three options when offered boons, but since this is from Hades who doesn't want you to win it would come with a heavy downside.
I was thinking losing a death defiance for the rest of the run everytime you do or flat out preventing players from respawning.
What do people think would be a balanced counter to such an ability?
