Can I request a LOA for this?
Post Body

So I was planning on moving to Corpus and trying to transfer stores. Spoke with several store managers in Corpus and they told me they'll only take me in immediately if I am actively taking classes at the local college there. So I deal with severe depression around this time of year and anxiety so that kicked in pretty bad. I've been giving away my shifts for the past month. I had a call from the secretary asking why I haven't been going in to work and I explained that it was due to severe depression and anxiety (my meds just aren't cutting it anymore). She apologized for my reasoning and said I needed to talk to my store director about a leave of absence. My store director has tried to write me up and fire me plenty of times for my appearance (mainly my "gothish" makeup and facial piercings) so I'm not fully comfortable talking to him. I called HR and my HR manager was out for the day but the secretary was in. She said I can at least talk to a store leader about it. So I called my store and spoke with one of them and she said that she's requiring my physical presence in order to get a leave and for me to explain my reasoning in person. I don't understand why I can't do it over the phone or email. Anyways, can I request a leave of absence for mental health reasons without a doctor's notice or do I have to have doctor documentation for any leave of absence. I'd really appreciate if someone could help me out here, I'm just really struggling mentally. And I still want to do that transfer as well :((
