At what point does medication come into play?

I realise this question might be a bit "How long is a piece of string?" But how many high readings / how frequently before the diabetes nurse will want to talk about medication? Is it like with glucose in your urine, where twice gets you sent for a GTT? Or more a case of "if your readings are over more often than not"? Thanks in advance!
