Currently on SSDI and wanting to fix my credit. Any advice?

Basically I was homeless for a couple years and finally moved back in with my parents and got on SSDI for my PTSD and ADD. I think I'm on the right medications, so I'm trying to fix my life, go back to school, get a job, etc. My credit score is horrible, though, (486) which is preventing me from getting a car or student loan or anything. I am about $5000 in debt with credit cards, payday loans, medical bills, and student loans (most of which are closed/charged-off). I only make about $950/month in SSDI. What's the best way to start working on paying off my debt and rebuilding my credit? I can't really afford more than $100/month.
