Trying to make a spike trap using monks active tile

So im trying to make a spike trap using monks active tiles and its mostly working. When a creature walks into the tile the image swaps to the spikes tile and it ask for its save and everything is good. The issue is that when the creature leaves the tile the process i repeated from the too. i dont want the trap to damage player when they leave it only when they enter it. but if i turn off the "on enter/exit" or the "on move" the token doesn't auto rest to it original state.

Any idea on how to fix??
