Getting back on the wagon

I was doing pretty good! Drinking water, eating better, sleeping, walking, yoga, down to 3 or 4 cigarettes a day. Life happens, blah blah blah, pack a day eating candy bars and swigging creamer with a dash of coffee. It's really hard trying to start over again! I've fixed a few of the problems but I'm having a hard time even setting real goals... Advice from those of you who've started over?
