Why did Dr.Sheaman call “Andrew” “Teddy” at the end of Shutter Island?

When Andrew/Teddy gets up to walk to the light house which the “hallucination” of Rachel was right about them doing lobotomies there Dr.Sheaman calls him “Teddy” right before it pans to the lighthouse! I don’t know if I can find a clip of that exact moment I surely can try I’ll be back if I find it.
Edit: They’ve “dropped the facade” and aren’t giving into his “delusion” anymore but Dr.Sheaman calls him by a “fake name”!
