sneak in combat, how does it work?

I know an action gives you the ability to skill test but does attacking at range break your sneak? Or would the opposing enemy have to break your success as a skill test? Example you roll 2 successes and one successful crit due to tag skill, would I have to contest that roll by trying to outskill your success? So I'd have to roll 3 successes on perception to see you?

If you attack then does that break it? Would. Suppressed weapon avoid you making noise?
