Making a replacement of a one-of-a-kind item?

So, I'm not on Etsy (yet) but I'd very much like to open a store after I'm finished school this spring. My question is, if you sell an item to someone, but you only have one of that item and it gets...damaged in transit, let's say, what happens then? You wouldn't have to make a replacement, correct? Or would it possibly depend on the item?

Sorry if this is a really dumb question, I'm probably overthinking things but what would you do in this situation? Refund the customer and have them go somewhere else? Try to make the item again?
