I did a quest on shoreline the other day, and was moving away from the luxury houses (the ones with the safes, close to village) towards the green house in the swamp village. Just before reaching the door, I start to take fire. I was hit severely, and three of my limbs was blacked. It was night time, so I could not see where it came from, but it sounded like it came from the field with the hay bales.

I decided to stay and wait, so I fixed all of my limbs and healed back up to full health, with no obvious sounds outside. I decide to try to run to extract, and I run as fast as I can out of the house towards the fence at the edge of the map. I take fire on my run there, but nothing hits. I run until my stamina is out, then continue walking. As my stamina is almost full again, I start taking fire once more and this time I am badly hit. A part of me wants to give up, but I popped a pain killer and started running.

This back and forth continued all the way to "road to lighthouse" but I got out with barely any XP. Tarkov is surely a good cardio exercise...
