Can someone explain how the Elgato HD60 S+ works, as far as hardware setup goes? And can I use it with only one monitor?

Like, how does everything hook up? Do I need any special equipment other than console, tv, and pc? I see people saying I only need one monitor and record through the recording software image, but does it hook up to two separate inputs in the TV where I could just start record, switch to the consoles direct input, and play without latency?
I legitimately cant figure out how it all hooks up, any help is appreciated.
