My mom went to the ER for chest pain and had a heart attack shortly after arriving. About 5 days later she had double coronary bypass followed by some complications that kept her in the hospital for 2 months. She didn’t even know who I was at one point. During her stay several dr’s seen her so her follow up appointments were daily with several dr’s. On top of not being able to file online because of some error no dr would send in the medical certification. Getting her mind back after the hospital stay it self was tough and keeping up with a sdi deadline she might not of realized. When she filed should she put down the date she had surgery or should she move the date forward ? Would suck for her to be denied for being late. Any thoughts on how to possibly move forward would be great.

Thanks!
