Did Molesley legitimately have breathing problems that would prevent him serving in WWI? Or did he take advantage of Clarkson? Molesley is generally honorable and he is shown out of breath running up to Downtown when the tails haven't been delivered for Matthew and also when Bates surprises everyone by coming back from the pub to work for Lord Grantham (thanks for the new shoe horn!)
I wondered if him helping with the soup kitchen is a way he decides to help with the war effort.
It's been awhile since I watched this episode but I've always understood the scene where Molesley is describing his symptoms to Clarkson to be him describing anxiety attacks/anxiety disorder, he just didn't know the term for it or how to describe the intense emotional state that accompanies it, pretty standard for the time. Clarkson understood what he meant though. A person with an anxiety disorder would not be a good candidate for the front lines, so Clarkson approved the exemption.
