NEED HELP.

My charger (2008 Dodge Charger 3.5L) makes a jerking motion after it goes into third gear every time I let go of the accelerator. I’ve already replaced the camshaft and crankshaft sensor but I’m still having the issue ( only happens when engine is warmed up, doesn’t happen when cold start). When I try to push it when it’s cold start it makes a a racketing noise around 60 mph. Can anyone help me and tell me what it could be?!
PS: it’s a P0016 code
