so I have a group of good friends and we are in the beginning of a campaign of my design. The first time we ever used this world i created i was doing a one shot with only one of the players, and his character had all his money stolen by a barmaid. Then when we played as a whole group the first barmaid they encountered poisoned them all. In an attempt at running princes of the apocalypse the barmaid at the tavern they all start in (which, by the way, is the official starting scene in that campaign. Like, really WotC? You couldn't think of something better?) Was caught tampering with the food. All of this was unintended. I actually didn't think about it, it just seemed convenient to use the barmaid as a plot point. Sooo, now my players dont trust any barmaid they come across.
