How should I implement Lycanthropy? [5e]

So I'm DMing for 5e, and I had my party encounter a wererat early on. A Goliath got bitten by it right before he died, and he is now infected with wererat Lycanthropy. I have a quest prepared for a cure, but if they decide not to do it, I'm gonna have him turn into a wererat within a weeks time.
So my question is, what should I have Lycanthropy do? I'm planning on having him automatically turn into an uncontrollable Wererat every full moon, but other than that, how should I implement him being able to turn into one at free will? What bonuses/disadvantages should I give him?
