So, I have a character concept that I've been working on for a new game and one idea I had would be a cleric that uses a spiked chain to disarm enemies from the cover of an obscuring mist. However, I'm not quite sure how the mist works. Is it basically like "If there is 1 square of fog between you and your target, they are partially concealed. If there is 2 or more, they are totally concealed"? Would I be able to land disarms from inside the mist? Are there any feats/abilities/spells that could help with making this work?
