Help finding a home brewed monster

So a while ago I stumbled across a home brewed monster that was posted with #ttrpg in Insta. I loved it and screen capped it so I could modify it/use it later.

And then the photo got deleted, and I of course can only remember parts of it.

On the off chance anyone here has seen it/created it/has used it and would be able to direct me to who created it that would be amazing.

So, what I remember: It was a large, undead creature. It was, I believe, tied to the moon in some way. It phased in and out of the material plane. It was swamp based, I think.

Not a lot to go on, but hey, can’t hurt to ask.
