I love the idea of examining a couple for fertility and breeding purposes. I'd examine each separately and then together.
I see a longer-term "getting to know you phase" to make everyone comfortable and then would get to know each partner individually. Their wants. Their needs. Then we'd have separate exams to begin. During each exam, they would need to have a head-to-toe inspection. Samples would be taken. This would allow the "doctor" to learn about each body allowing me to understand how to use the partner's body to best create sexual pleasure.
Then, the doctor would be present as the couple "bred" offering advice, perhaps even physical adjustments if needed, though the doctor's penis would never come out.
Really this is about a third-party assisting a couple in creating intimacy and anticipation in a safe, caring, and fun environment.
It's certainly a very deep take on the medical fetish, but one I would love to chat out sometime. Not really looking for role play
