I love the idea of examining a couple for fertility and breeding purposes. I'd examine each separately and then together.
During each exam, they would need to perform some various exercises for me before finally being asked to try and "procreate" exactly how the doctor asks.
It's certainly a very deep take on the medical fetish, but one I would love to live out sometime.
